Here are the essential pieces of information we need to fill your order:
- Your name, phone number and email
- Delivery address
- Patient’s name - please include phone number and email if we are charging the patient directly.
- Labeling instructions - especially important if you are ordering more than one formula at a time for the same patient. For example “Luteal” and “Follicular” or “Day Time” and “Night Time”
- Number of refills. This is the number of times a patient can re-order from us directly without going through the practitioner. Most practitioners choose zero for acute conditions, and 1‐2 for chronic maintenance conditions.
Special Notes for Raw Herbs and Vacuum Pack
- List each herb with daily dose
- Include any special instructions for individual herbs. For example
- “Cook for __ minutes first before adding other herbs”
- “Add during last __ minutes of cooking”
- Crush
- Bag separately
- Note: during vacuum packing all the herbs are cooked together at once. Essential oils from short‐cook aromatic herbs are preserved in the sealed cooking unit, while the high temperature extracts all the necessary active ingredients from shells and minerals in the allotted time.
- How many days you are ordering (one pack of raw herbs = one day). The minimum order for vacuum packed herbs is 5 days, but there is no minimum order for raw herbs. You can even get one bag of raw herbs.
Special Notes for Granule Herbs
- Minimum order is 100 grams
- You can write your granule formula as you write a raw formula - we can easily convert to percentages and preserve your ratio.
- We do offer encapsulation
